These terms govern your use of PhacoBook. By creating an account or using the service, you agree to them. If you do not agree, do not use PhacoBook.
The service
PhacoBook is documentation software for cataract surgeons. It lets you record structured operative data, run training evaluations and patient-reported outcome measures, and view metrics — including anonymous, aggregated benchmarks drawn from a shared registry.
Not a medical device
PhacoBook is a documentation and registry tool. It records and summarizes the information you enter. It does not make clinical decisions, provide diagnoses, or give medical advice. Clinical judgment and responsibility for patient care remain entirely with you as the treating clinician.
Your account & acceptable use
- You are responsible for the accuracy of the data you enter and for keeping your credentials secure.
- You must have the appropriate authority and legal basis to record and process the clinical data you enter.
- You must pseudonymize patients — do not enter directly identifying patient information into clinical data fields.
- You must not misuse the service, attempt to de-anonymize registry data, or use it unlawfully.
Your data
You retain ownership of the clinical data you enter. You may export your full record and delete your account at any time. By using the shared registry you agree that anonymous, aggregated statistics derived from your cases — which cannot identify you or your patients — may be used to provide community benchmarks. How we handle data is described in the privacy policy.
Availability
We aim to keep PhacoBook available and reliable, but the service is provided "as is" without warranties of any kind. We may modify, suspend, or discontinue features from time to time.
Limitation of liability
To the maximum extent permitted by law, PhacoBook is not liable for any indirect or consequential loss arising from your use of the service. Nothing in these terms limits liability that cannot be limited by law.
Changes
We may update these terms from time to time. Material changes will be reflected here with a new "last updated" date. Continued use after a change constitutes acceptance.
